Community Health Improvement Plan
A community health improvement plan is a systematic, long-term effort to address key public health needs and issues in a community. It is based on the results of the community health needs assessment activities, through comprehensive data collection and analysis.
Wise Health System has developed a community health improvement plan, with collaboration from community steak holders, to define and set priorities for a vision for Wise County’s community health. It contains goals, objectives, and strategies to improve the health of Wise County.

Community Health Needs Assessment
As part of the Affordable Care Act, non-profit hospitals must complete a community health needs assessment every three years. Hospitals must collect input from the community, adopt a plan to address community needs and then report on these activities.View Publications
The purpose of the community health needs assessment is to understand the needs unique to those in the community served by Wise Health System and to document compliance with federal laws.

Community Health Needs Model
Wise County is faced with a great opportunity to improve the overall health of each citizen. Wise Health System presented a format called the Community Health Improvement Model in response to the Community Health Needs Assessment needs identified for 2017. These needs included Obesity, Lack of Mental Health Services, Lack of Access to Services, Diseases of the Heart, Physical Inactivity, Diabetes, Poor Nutrition, Transportation and More!
We categorized each of these needs into the committees below who will each identify a measurable outcome and create a strategic plan to improve each need. We will provide all data necessary to create these strategic plans as well as support to the committees overall.
Partner with Us
We are asking for partners in this initiative to join a committee and adopt the principles of the Community Health Improvement initiative. We encourage everyone to commit to be a partner, support the mission, and promote the activities of Wise Community Health Improvement. For example, you can encourage employees and cohorts to lead healthier lives, form a group and participate in Walk Across Texas or start the conversation about health with those around you. All of our partners will be published in the strategic plan for the initiative for all of Wise County to see.
If you are able, please identify which committee you would like to participate in. An executive committee will also be formed to look at the project overall and collaborate on any issues the individuals may be facing.
