What is HealthConnect?
HealthConnect is a secure, easy-to-use, online patient portal developed by Wise Health System to provide 24/7 access to your personal health information.
Why is HealthConnect important?
Accessing your personal health information through a patient portal can help you be more actively involved in your own health care by becoming more informed. You will be able to access your Summary of Care record that you can view and download for your records or to share with your primary care provider for improved quality of care.
What information is available in HealthConnect?
Inside HealthConnect, you’ll be able to view your Summary of Care, which provides a summary of your hospital visit: current health issues, medications, laboratory and radiology reports, allergies, recent medical history, and other select pieces of personal health information. This Summary of Care is available in the portal within 36 hours of discharge.
Who is eligible for a HealthConnect account?
All patients seen at a Wise Health System facility on/after July 1, 2014 through May 2, 2019 are eligible for a HealthConnect account. Inpatients will notice more depth of information available on the Summary of Care due to the nature of services provided.
Can my child have a HealthConnect account?
At this time, only patients 18 years of age and older will be able to register for a HealthConnect account. We currently don’t offer the ability to link a parent’s account to a minor.
Can I prevent my information from being shared?
Yes, you may opt out during the registration process. If you choose to opt out, your information from that visit will not be available in the portal and you won’t be able to log-in and view your Summary of Care.
Is my information secure?
We do not sell or rent any personal information we receive through your use of HealthConnect. To make sure your private health information is safe, HealthConnect is hosted on a secure connection and accessed via an encrypted, password-protected logon using SSL.
Who can access my information?
If you are transferred or referred to a provider who participates in HealthConnect, that provider will be able to log-in and view your Summary of Care record in order to improve your care. Other health care providers that join HealthConnect will only be able to view your Summary of Care record by utilizing a very strict “break the glass” method of search. This method requires the provider to specify a legally permissible reason for accessing your record in order to prove a previous relationship exists between you and that provider.
How do I register for a HealthConnect account?
Please visit our online registration form to request access to the HealthConnect patient portal. Registration is easy!
Patient portal access computers are located in the Health Information Department at the main Decatur East Campus across from the Emergency Department. These computers can be used to register for, and access HealthConnect.
Can I communicate with my physician in the HealthConnect patient portal?
Direct messaging to your physician is currently not available from inside the portal, but is a feature that will be available in the near future. Please contact your physician’s office directly with any health-related questions.
Is HealthConnect the same as the patient portal I use at another doctor’s office?
No. Many other physician offices have their own patient portals that are completely separate from Wise Health System’s HealthConnect.
