Our seven OR suites have the most advanced technology to ensure our patients have access to the most innovative surgical treatments and procedures available today.

How to Schedule a Preop Assessment and Surgery

  • To schedule preop assessment and surgery, call (940) 626-1730 or fax the scheduling sheet to (940) 626-2559.
  • Confirm surgery date and preop assessment appointments with patient.
  • Give patient their preop assessment letter and home medication list.
  • Fill out preop demographics, orders and insurance pre-cert form.
  • Fax ALL preop demographics, pre-cert, orders and H&P to Admissions at (940) 626-1718.
  • Fax preop demographics form to (940) 626-2559 (this is to ensure that we have any surgery specific equipment, implants or instruments needed for the procedure).
  • Call the assistant surgical director at (940) 626-2556, if you have any questions.
  • NOTIFY PATIENT AND SURGERY SCHEDULING IF PROCEDURE IS RESCHEDULED OR CANCELLED.
